Obtaining Medical Records

Every health care provider maintains patient records. The medical record includes forms, questionnaires, providers’ notes, laboratory or other testing results, notes from phone calls, and communications with others regarding your child.

Access to medical records is protected by Pennsylvania State Department of Health regulations. Copies of records may be released upon receipt of written request from the patient (if 18 years of age or older) or the parent. There may be a charge for this service unless the copies are requested to maintain continuity of care.
