Finding a "Medical Home"

A "medical home" for children with special health care needs is not a building, house, hospital, or set of services, but rather an approach to health care that is:

  • accessible
  • continuous
  • comprehensive
  • family-centered
  • coordinated
  • compassionate
  • culturally effective

With the "medical home" approach, a pediatric care team partners with the child and family to assure that all of the patient's medical and non-medical needs are met.

Some primary care practices have obtained training in the "medical home" approach. But even if your child's primary care provider has not received the training, ask the about the provider's ability to coordinate your child's care. Many primary care providers support families in this way.
